According to statistics stated by the International Society of Aesthetic Plastic Surgery ISAPS, liposuction surgery is the second most popular cosmetic surgery in the world.

Based on the statistics released by the agency in 2019, Liposuction surgery ranks second after the augmentation mammoplasty by a very small gap, with over 1.7 million surgeries worldwide each year.

This number has increased by 20 percent within the last 4 years alone.

You have probably seen athletes who are dissatisfied with the mass of fat in some parts of their body which they cannot get rid of by dieting and exercise.

The liposuction surgery is done with the objection of getting rid of the STUBBORN FAT in separate parts of the body in a way of achieving an ideal body form.

In ladies, these body parts include the thighs, the stomach, the back, the sides of the stomach, and the arms. In men, they mostly include the stomach and the sides of the stomach.

Generally speaking, in this method, the excess fat is sucked out through a thin tube called a Cannula which is entered into the underlying fatty tissue through a small cut in the skin.

In all liposuction methods in Iran, a general technique that consists of making a small cut in the skin and using a thin tube called a cannula to remove the excess fat under the skin is practiced.

Based on the area that the surgery is to be performed on, the mass of the accumulated fat, and the cost, the intended technique is picked out.

Also, depending on the area where the liposuction surgery is to be performed and the patient’s preference, local anesthesia or general anesthesia is used. Local anesthesia is usually used to remove fatty tissues with a volume of less than half a liter.

• The procedure is carried out without the need for anesthesia.
• First, a solution containing a local anesthetic & a drug called epinephrine is injected into the intended treatment.
• This solution fills the fat layers of the skin, making the adipose tissue swollen.
• The epinephrine in this solution also causes temporary shrinkage and tightening of blood vessels, and by reducing blood flow.
• After injecting the solution, the surgeon makes a small incision in the skin & inserts a cannula into the layer of adipose tissue.
• Using back and forth motions, the fat is sucked through the cannula and into a sterile deposit system.
• The surgical process itself is still quite long and lasts approximately 3 to 4 hours.
• Also, the amount of adipose tissue that is normally extracted is approximately 4 to 5 liters. (Venkataram, 2008)

A very interesting concept exists behind the use of this technique of liposuction. It is a fact that the mere force of water in nature is able to make changes. In surgery, the same water force can help make changes in the shape of the patient’s body.

Water jet liposuction separates fat cells from other tissues and causes the least amount of damage to the patient. It is worth pointing out that the adipose tissue is not damaged during the procedure. This means that it is possible to use the fat for grafting in other parts of the body that need fat. In ultrasound-assisted liposuction, ultrasound energy is used to break the bonds between the fat cells that have accumulated in the body. Thus, adipose tissue turns into a liquid state and is then removed from the body.

The use of ultrasound waves can be conducted both before the surgery (from outside the skin) and after surgery (with the help of a cannula tube under the skin). This method is more applicable in cases where the mass of fat is more resistant and stubborn.

Among the devices of ultrasonic liposuction techniques, the VASER device is more popular.

The VASER (Vibration Amplification of Sound Energy at Resonance) Liposuction technique, is an advanced variation of ultrasound-assisted liposuction.

The VASER method is unique owing to the fact that it allows the surgeon to separate and extract fat from the body with high precision and delicacy, without causing any damage to other tissues.

This medical approach may be performed on various areas of the body, including the arms, breasts, chin & neck, upper back, waist & abdomen, upper thighs, thighs, and buttocks.

Power-Assisted Liposuction, which is also known as PAL, is one of the best ways to get rid of localized masses of stubborn fat that prove to be very difficult to eliminate by dieting and doing exercise.

This method can be used on various areas of the body such as the thighs, sides, calves, breasts, and abdomen. The way forth is that the Tumescent solution is first injected topically into the intended area to swell the tissues and make it easier to separate the mass of fat.

Small incisions are then made in the area, and a thin hollow metal tube (cannula) is inserted into the dense adipose tissue. There is a vibrator at the tip of the cannula, which loosens and separates the fat tissue with its gentle movements back and forth. Then, the fat is sucked out through the hollow part of the tube.

This method is utilized for areas of the body that are more delicate such as the face and the neck.

In this method, a laser is used to break down the mass of fat and then the suctioning is performed. This technique helps prevent skin loosening after liposuction in Iran as it stimulates the collagen found in the skin tissue.

There are three techniques for liposculpture in Iran in general. The kind of technique that is suitable for you depend on different factors including the area situation that is under treatment and the amount of fat. You can choose the most appropriate one consulting your physician

• Liposculpture by electricity (PAL)
• Liposculpture by ultrasound (UAL)
• Liposculpture by laser

Studies have indicated that complications and side effects after liposuction surgery are about 5% and minor, which indicates low risk for this surgery.

On the other hand, out of every 5000 liposuction surgeries, one has resulted in the death of the patient, which points toward a risky surgery. What is the reason behind this?

Generally speaking, the risk of liposuction is significantly increased if the surgical examination of the appropriate physiological conditions, excessive removal of fat mass, or multiple concurrent surgeries is not assessed properly.

The ideal weight for this aesthetic surgery is having a body mass index (BMI) of less than 30 and appropriate physical conditions.

If you do not have the proper lean muscle mass or suffer from a particular illness such as cardiovascular disease or diabetes, the risk of the surgery will probably be high for you and your surgeon should not perform the operation under such conditions.

The most common of liposuction complications are skin roughness, bumps, dimples, sagging skin, numbness, infection, sores, and scarring.

The continued effect of local anesthesia after surgery greatly diminishes post-surgery pain.

As the anesthetic wears off, most patients experience some degree of pain, that is usually controlled by prescribing Tylenol. (the University of California, San Francisco Medical Center, 2020)

Physicians encourage walking immediately after surgery, owing to the fact that doing so speeds up the healing process. You can resume doing exercise 3 to 7 days after receiving your liposuction. If your job does not involve doing heavy physical work, you may be able to return to work in just a few days.

Most of the inflation will subside within a few days. Although you will observe a great amount of improvement within a week or two, it can take a minimum of 3 to 4 months (even up to 6 months in some cases) to obtain the full results and positive outcomes of the Liposuction.

Most patients acquire the desired results with just one session of liposuction. After a mere week, the results of the treatment will be evident. Achieving the final and permanent result of the treatment may take up to 6 months.

The results obtained from liposuction treatment will last longer by having a healthy lifestyle.

A 2010 study found that 80 percent of the 609 patients who had undergone the procedure between 2002 and 2008 were satisfied with the outcome of their treatment.
