What is chin surgery (genioplasty)?
Mentoplasty or Genioplasty or Chin Surgery is an aesthetic surgery that is used for chin enlargement (forward movement) or reduction (backward movement), shortening or enlarging, or enhancing the chin. Some people have asymmetrical chins; based on the chin size, a chin implant (to fill the free space of the chin and chin augmentation) or chin reduction may be considered to achieve chin symmetry.
Although some chin deformities are caused due to dental malformations, which may be modified by orthodontic treatment, thus for proper treatment, it is vital to first diagnose the problem.

Chin Surgery Types
Genioplasty surgeries are divided into two main groups:
Chin Augmentation (Chin Implant) Surgery
Chin augmentation surgeries in which small or retracted chins are enlarged to moved forward.
Chin Reduction (Chin Shaving) Surgery
Chin reduction surgeries in which large or protected chins are reduced or moved backward.
Different Methods of Chin Reduction Surgery
Sliding Genioplasty (Osseous Genioplasty)
In this method, the intraoral part of the lower lip is incised and the adjacent soft tissue is pulled over. Then, the chin bone is cut horizontally. Then, the bone is moved backward and the is stabilized with screws. A metal plate may be also used to reassure the bone non-movement. Lastly, the sharp bone edge should be shaved. This surgery is suitable for those with relatively protracted chins.
Direct Chin Reduction Surgery
Horizontal chin movement is not required in this method. The intraoral part of the lower lip is incised and then a part of the chin bone is safely and effectively excised in the length or width. Minor asymmetries may be modified with this operation.
Chin Augmentation Techniques
Chin Implants (Chin Prosthesis)
This surgery is done for chin shaping, enlarging, and protraction. This operation may be done using an intraoral incision below the lower lip, or an external incision below the chin. Then the prosthesis is placed on the shin bone. The intraoral wound will be healed within 5-7 days. This operation may be done under general anesthesia or local analgesia. The final outcome will be a permanent natural chin.
Sliding genioplasty (Osseous genioplasty)
This method may be used for both chin retraction and protraction. In the chin protraction (augmentation) surgery, the intraoral part of the lower lip is incised and the adjacent soft tissue is pulled over. Then, the chin bone is cut horizontally. Then, the bone is moved forward and the is stabilized with screws.
Fat Transplant to Chin Augmentation
In this method, fat tissue is obtained via different methods of liposuction for the patient’s own body. Then, the healthy fat tissue is injected into the chins. Although the fat will be partially absorbed, the rest of the fat will be permanent.

Comparison of Surgical and Non-surgical Chin Augmentation
Nonsurgical chin augmentation methods may be used which involve filler injection to specific areas in the chin. Filler injection will have a durability of months to two years based on the type of filler. Non-surgical methods are suitable for those who are not willing to undergo surgery but are willing to have a prettier appearance.

Recovery after Genioplasty

The swelling and bruising after the sliding genioplasty will last for 6 weeks or more when you can see the permanent final outcome. Note that the chin tissue will be affected by aging and the chin shape may change.
The initial outcome of the chin surgery may be seen after one month, though the final outcome will be evident after three months when the swelling is completely removed.
